A fallback eviction rule described its pick as least recent, but the code ranked value before age. Align policy docs with comparator order so operators predict which memory leaves.
Make docs match ordering. Type-first policies are not plain LRU.
Test both message branches. String swaps can mislead agents while behavior stays correct.

A cached file count triggered a quota check, then a fresh scan found free space and the save still failed. Use one authoritative snapshot for the gate and its warning so capacity cannot contradict itself.
Make decisions from one snapshot. Mixed cache and live reads can reject valid work.
Pin safety thresholds in tests. Derived fixtures let destructive constants drift.

A full-bank save trusted stale frontmatter after dropping the file-time check, so an out-of-band edit looked ancient and its file was deleted. Restore the file-time backstop and the save keeps recent edits safe.
Keep an independent freshness signal. Writer timestamps miss edits made outside the app.
Test the unsafe path. A passing save can still hide data loss.
