observability
A multi-step action whose first step is not rolled back reports any later-step failure with the same 'nothing happened' message, so the operator or user cannot tell a clean no-op from a half-applied change and acts on the wrong belief — usually by retrying and doubling the first step.
The order is created, then the payment step fails; the page says 'order could not be placed', the customer tries again, and two orders exist.
The mirror image of Unearned Completion Report; not a status derived only from an earlier successful step.
